This service is for individuals living and working in New Zealand who have experienced mild illness and need a medical certificate for their workplace. If your illness or injury is outside a pharmacist’s ability to assess, they will refer you to a doctor or other relevant healthcare professional.
We offer telehealth consultations for medical certificates for up to 3 days. For extended time off work, a physical examination by your regular clinician is required.
If you're unable to work because you are sick, your employer can ask you to provide a medical certificate.

- Attend the consultation using the link sent to your email or phone. Please ensure that you give a complete and accurate description of your reasons for requesting a certificate.
- If you meet the criteria for provision of a medical certificate, we'll send you a link to complete your payment. The fee is $34.99
- Once payment has been completed, we'll email you your certificate
Any information collected during the consultation will remain confidential and will not be shared or used for any other purpose without your consent.
If a pharmacist is unable or unwilling to issue a certificate, and you believe you are unfit for work, you should consult a medical practitioner.
A pharmacist consultation is not intended to replace medical treatment or diagnosis. If your illness or injury persists or worsens, see a doctor.
- Chemist Warehouse is unable to issue certificates for periods of more than 3 days. In cases where a pharmacist considers a person may require longer periods off work, you will be referred to a medical practitioner
- No certificates are to be issued for any staff member employed by the Chemist Warehouse group. This includes all stores and support office in New Zealand
- Pharmacists are not able to provide certification for compassionate leave
- We are not able to provide certification under ACC medical certificates
